Senior Social Worker (Care@Home) (534390)

• Deliver accessible and equitable social work services to adults of the care@Home program.

• Provide an effective, holistic psychosocial service that is evidence-informed, locally appropriate and sustainable.

• As a senior member of the health care team provide support to leadership in developing, implementing and monitoring policies and practices that reflect the priorities and standards of the service.

• Extensive experience as a practising social worker, together with substantial knowledge and skills in the provision of a Social Work service in hospital and community settings and sound knowledge and understanding of the principles of primary health care and of current state and national health vision and policy.

• Proven high level communication skills, including the ability to work collaboratively as part of an interdisciplinary health care team and to build positive working relationships with other service providers, local support organisations and the wider community.

• Ability to provide senior support and social work intervention of chronic health needs, knowledge of community resources and the ability to provide support to health professionals, non-government service providers and the wider community in relation to health care needs.

• Fixed term full time, day worker position, working 76 hours per fortnight, commencing as soon as you can join us until 30 June 2028. *Notwithstanding hours to be negotiated with the successful applicant.

Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.

$117,272 to $126,182 per annum (Pro rata). Our Employer 12% superannuation contribution is on top of this amount.

• Salary range is in accordance with Allied Health Professionals Public Sector Union Wages Agreement 2025

• Please note that access to salaries beyond $123,487 is subject to qualifications and/or application to the personal upgrade scheme

In addition, the following allowances calculated on the salaried incremental point may be available:

• Professional Development Allowance of up to $1200 per annum, paid on a fortnightly, pro rata basis.

• Tertiary qualification/program of study approved by the Australian Association of Social Workers (AASW)

• Eligible for membership with the Australian Association of Social Workers (AASW)

• Overseas qualified Social Work candidates require a "certification of eligibility" letter from the AASW to confirm their eligibility for membership to the AASW

• Or where regulatory requirements exist, NDIS Q&S Commission Provider Registration Requirements

• Full member of Australian Association of Social Workers

• Current Working with Children Registration

• Current Driver's Licence
