Job summary A Care Navigator is a very special person! They are the first port of call for patients and they respond to requests for GP appointments, made by patients either on the phone or in person. With training and support, they will triage the patient non-clinically, and signpost to the appropriate service or person, which may not necessarily be the GP. They will work in a reception hub along
