Registered Nurse - Patient Aligned Care Team

Join the VA Black Hills Health Care System in Rapid City, South Dakota, as a Patient Aligned Care Team Registered Nurse Care Manager. Coordinate proactive, Veteran-centered primary care for a designated patient panel, support chronic disease management and care transitions, provide health coaching and education, and collaborate across disciplines to improve access, continuity, wellness, and long-term health outcomes across in-person and virtual care settings. U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
