Nurse (Case Manager HUDVASH)

The Housing and Urban Development- Veterans Affairs Supportive Housing (HUDVASH) Registered Nurse (RN) Case Manager executes position responsibilities that demonstrate leadership, experience, and creative approaches in providing complex patent management. Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings.
