Part-Time Nurse Case Manager
2.5 days per week
Monday through Friday, 8:00 a.m. to 8:00 p.m. EST
What You'll Do
- Serve as a primary point of contact for patients enrolled in the Patient Support Program.
- Provide education regarding prescribed therapies, treatment expectations, medication administration, and adherence strategies.
- Support patient enrollment and onboarding activities.
- Coordinate care and communicate with physicians, clinic staff, specialty pharmacies, and other healthcare professionals.
- Assist patients in navigating access to treatment and reimbursement processes.
- Conduct patient assessments and follow-up activities as required by the program.
- Monitor patient progress and provide ongoing support throughout the treatment journey.
- Maintain accurate and timely documentation within program systems.
- Identify and escalate clinical or operational concerns as appropriate .
- Contribute to quality improvement initiatives and program success measures.
- Build and maintain positive working relationships with healthcare providers and clinic partners.
Basic Requirements
- Active and unrestricted nursing license in
Alberta and/or British Columbia
(RN, RPN/LPN, or equivalent nursing designation).
- Diploma, degree, or equivalent combination of education and experience consistent with the
P1 Professional Career Framework
.
- Strong verbal and written communication skills.
- Proficiency using Microsoft Office and electronic documentation systems.
- Ability to work approximately
2.5 days per week
8:00 a.m. to 8:00 p.m. EST, Monday through Friday
.
- Ability to travel to clinics and healthcare sites throughout Alberta and British Columbia as required .
- Excellent organizational skills and attention to detail.
Preferred Skills & Experience
- Experience working within a
Patient Support Program (PSP)
.
- Prior Nurse Case Management experience.
- Experience supporting patients receiving specialty medications or biologic therapies.
- Knowledge of reimbursement, patient access, or specialty pharmacy processes.
- Experience providing patient education and clinical support.
- Experience collaborating with physicians, specialty clinics, and multidisciplinary healthcare teams.
- Familiarity with chronic disease management and treatment adherence programs.
- Additional active nursing licenses in Canadian provinces.
Travel, Work Environment & Physical Requirements
- Primarily remote/home-based role with travel as required .
- Travel to clinics, healthcare facilities, and stakeholder meetings throughout Alberta and British Columbia.
- Valid driver's license and access to reliable transportation may be required based on territory coverage.
- Must be able to work independently in a remote environment while effectively managing competing priorities.
- Occasional lifting and transportation of educational or program materials may be required .
