Manager, Population Health - Stars & Quality Programs

Specific responsibilities include: • Key focus on health outcomes and controlling medical costs in our patient population • Owning the strategy, process, and implementation of key operational roll-outs (will involve on-the-ground support at our clinics) • Leading the clinical development of new care programs, including coordinating and synthesizing feedback from care teams and reviewing evidence-based guidelines • Reporting and analytics in support of our Care Teams (physicians, nurses, care managers) • Establishing and maintaining relationships with both internal and external stakeholders • Other duties, as assigned The role reports to the Director of Population Health Strategy & Analytics and has significant @@HD@@ opportunity for rapid advancement.
