• Performs initial and ongoing patient assessments based on patient needs and physician orders. • Develops and updates a written plan of care, ensuring it is re-evaluated as necessary. • Provides skilled nursing care within the nursing scope of practice. • Documents care and services in compliance with federal, state, and Agency regulations. • Leads case conferences to discuss multidisciplinary team responsibilities, patient progress, and future care plans. • Supports patient/family cultural practices, provided they don't interfere with medical treatment. • Promotes patient, family, and caregiver education tailored to cultural, ethnic, and personal needs or preferences.
