: Evaluate patients for functional limitations and collaborate with patients, families, nurses, physicians, and other health team members to develop a personalized therapy plan. •

: Provide therapy services to treat medical and surgical conditions using accepted physical therapy modalities. •

: Instruct and supervise patients, families, home health aides, and others on home exercise programs and aspects of patient care. •

: Continuously assess the patient's condition and adjust the therapy plan as needed. •

: Report significant clinical information to physicians, clinical managers, and other team members. Document clinical evaluations, treatments, and patient responses to therapy.
