As a Case Manager, you will: • Conduct in-home, face-to-face assessments using comprehensive tools for members enrolled in Managed Long-Term Services and Supports (MLTSS) and/or Dual Special Needs Programs (D-SNP/FIDE). • Evaluate member needs and coordinate services such as adult/pediatric medical daycare, personal care assistance, nursing facility placement, and more. • Collaborate with members, families, providers, and interdisciplinary teams to develop and implement individualized care plans. • Advocate for members during discharge planning and ensure safe transitions with appropriate services in place. • Document assessments and care plans accurately and timely in the electronic health record. • Mentor new hires once proficient in your role.
